Job Title: Home Health Registered Nurse-RN, PRN

Job Summary:
We are seeking a dedicated and compassionate Registered Nurse to join our home health care team on a PRN (as-needed) basis. The ideal candidate will be responsible for providing skilled nursing care to patients in their homes, ensuring their comfort and safety while promoting their independence. This position requires a high level of autonomy and the ability to assess and address the needs of each patient in a thorough and compassionate manner.

Responsibilities:
1. Conduct initial patient assessments and develop individualized care plans
2. Administer medication and treatments as prescribed by the physician
3. Provide wound care, IV therapy, and other skilled nursing services
4. Collaborate with the interdisciplinary team to coordinate patient care
5. Educate patients and their families on disease management and self-care techniques
6. Document patient care and maintain accurate medical records
7. Participate in regular case conferences and treatment team meetings

Qualifications:
1. Valid RN license in the state of practice
2. Minimum of 1 year of clinical experience in a home health or similar setting
3. Strong assessment and critical thinking skills
4. Excellent communication and interpersonal abilities
5. Ability to work independently and manage time effectively
6. Knowledge of Medicare and Medicaid regulations
7. Current BLS certification
